Death Certificate

The following information will be needed to complete the death certificate. It typically takes 4 – 8 weeks to received certified copies of the death certificate. 

  • Full name and address of your loved one 
  • Date of birth 
  • City/County of birth 
  • Parents’ names (please include mother’s maiden name) 
  • Marital status – must be verifiable
  • Family Names (Husband, Wife, Children, Grandchildren, Brothers, Sisters etc.) 
  • Social Security Number 
  • If your loved one is a Military Veteran, please bring discharge paper ( DD 214 ) 
  • Years of education 
  • Occupation and Employer 
  • Cemetery information 
  • Number of certified copies of Death Certificate 

Obituary

  • Type your obituary using Microsoft Word 
  • Email the obituary to your funeral director at the email address that he provided to you.
  • Attach 1 jpeg picture that you wish to publish with the obituary. Do not place in body of email.
  • Do not send the obituary or picture as a PDF attachment.  Must Be Word Doc/Docx and JPEG
  • Feel free to use any obituary found at www.easttexasfuneral.com as a guide. 
  • We will not accept hand written obituaries. This could lead to errors and misprints.
